What Is SIBO?
Bacteria are generally found in large intestines, where they perform various symbiotic functions for the body. SIBO is a condition where bacterial overgrowth occurs in the small intestines, typically producing following symptoms:
- Bloating and /or wind
- Abdominal pain and cramping
- Halitosis
- Burping and Acid Reflux
- IBS diarrhoea/constipation
- Food sensitivities
- Joint pain
- Skin rashes
- Temporary improvement in symptoms after antibiotic therapy
What May Contribute To Development Of SIBO?
- Previous gastrointestinal infections such as traveller’s diarrhoea, food poisoning, gastroenteritis
- Overconsumption of simple carbohydrates
- Medications (e.g. proton-pump inhibitors (PPIs), opiates, prolonged use of broad spectrum antibiotics (e.g. amoxicillin, tetracycline)
- Traumatic brain injury
- Chronic stress, which decreases stomach acid production
- Ileocecal valve dysfunction, which results in a back flow of material from the colon into the small intestine
- Adhesions and scarring due to surgery
Diagnosis Of SIBO
There is currently no single gold standard test for SIBO diagnosis, however, the most commonly used procedure is a lactulose breath test (LBT). This involves the administration of 10g lactulose, with breath samples collected at 20-minute intervals for three hours.
The intestinal flora ferments the testing substance, resulting in the production of hydrogen and/or methane (depending on the overgrowth type), and it is these gases that are measured.
Glucose together with lactulose breath test may be a more accurate method for assessing the presence of SIBO, due to lactulose being useful to identify overgrowth in the distal small intestine, while glucose is more useful in identifying overgrowth in the proximal parts (due to the site of glucose absorption).
